尊敬的校領導：

　　我是我校X級會計二班的學生夏秋葉。我在去年的一次體育課上，由于不慎摔了一跤，造成了左腿骨折。經過一年的治療和調養，現已基本痊愈，為了不耽誤下學期的課程學習，現特提出申請，請求復學。

　　去年住院以后，由于不能上課，就向學校提出了休學申請。當時我的輔導員張治業老師跑前跑后，所有的休學手續都是他給辦的，因此有些具體復學的手續我還不太清楚。上周，我曾給張老師打電話，但辦公室的老師告訴我張老師已經調往別的單位了。我也不好再麻煩張老師。我開始休學的時間是6月7日，再有20天正好就是整一年。由于學校進入6月份，臨近期末，各方面工作較多較忙，又快放暑假了，因此為了能在學校放假前將所有的入學手續辦妥，我就向學校教務處的王老師作了咨詢。王老師給了我很多幫助，我已填好了有關的一些表格。

　　在家修養這一年中，我從未放棄過自己的課本學習。出院不久，我就讓家人把我所有的專業書籍從學校拿了回來，每天給自己制定了具體的學習計劃。專業外語，我每天幾乎都在讀。班上同學來我家時，我就會把看不懂的地方拿出來，向他們求教。這一年來，我還讀了不少提高個人修養方面的書，讀了一些中外名著。因此，一年來我雖未在校學習，但卻并未停止學習。因此，我希望領導考慮能否讓我重新跟原班學習。我知道這種提法有些過分，但我請學校找有關的老師考試一下，再做決定。若我各方面知識確實不讓人滿意，我將無怨無悔重新學起。
